Care Management provides efficiency and flexibility through our client-centered approach. What is a Care Manager? A Care Manager is a professional who specializes in assisting clients and their families with long-term or short-term care arrangements, as well as those with disabilities.
Who uses Care Management? Clients who prefer not to burden family members, family members who live out of town, family members with time constraints and multiple obligations, clients and families facing complex health needs with limited medical knowledge.
How can Care Management help? We act as a liaison to families at a distance making sure things are going well and alerting families to problems. We assist with recommendations and coordination for a client to or from a retirement complex, assisted living facility, nursing home or their own home. We provide consumer education and advocacy by offering counseling, information and crisis intervention.
Patient advocates are hard to come by in today's medical community. We will be available when it is not always possible for family members to be there. Advocates for the Family, Care Management program will provide resources, guidance and emotional support needed with these delicate matters.
Services may include: Individual Assessments Consultation and planning for the client's needs Customized home care services Private hiring and placement services Transportation, errands and telephone reassurance Communication of information to family members or physicians Assistance with applications for insurances, admissions, etc.

Care Advantage has built its reputation on providing quality health care for patients at home however, we can also help when family members are in the hospital, nursing home or assisted living facility. Some patients may need extra attention to make their recuperation, stay or transition quicker and easier. Some families want the reassurance of constant supervision of their loved one either during the day or at night. Qualified Nursing Assistants and Companions can assist with:
Bathing Dressing Mealtimes Walking or changing positions Companionship Supervision Transportation, outings and errands
Your loved ones will receive personal one to one care and attention with our Private Duty 1 to 1 Services.
______________________________________________________ Many times health care services that are provided in the home are covered by health insurance plans. Our office staff will investigate your insurance plan to determine your benefits and coordinate physician orders, as well as handle insurance billing. Potential placements are thoroughly screened to include a personal interview, background evaluation including professional and personal references, criminal history and sex offender check with the Virginia State Police, health certificate or negative tuberculin skin test results, employment eligibility verification, CPR verification, if applicable, skills testing and license verification with the Virginia State Board of Nursing, when applicable.
